Dr. Mitwally is a reproductive endocrinologist and infertility specialist and a Fellow of the American College of Obstetricians and Gynecologists (FACOG). He is triple-certified by the American Board of Reproductive Endocrinology and Infertility, American Board of Obstetrics and Gynecology, and American Board of Bioanalysis. He is internationally known for developing a new infertility treatment using the aromatase inhibitor letrozole and holds several patents for its use in infertility and other reproductive conditions. In 2019, ASRM (American Society for Reproductive Medicine) selected Dr. Mitwally’s research on letrozole in fertility treatment as one of 25 landmark papers from the previous 75 years. Originally from Egypt, Dr. Mitwally operates Mitwally Fertility Clinic with his wife and four sons, with offices in San Antonio, Texas, and Columbus, Ohio.

Dr. Mitwally completed medical school in Egypt in 1990 at the top of his class. After an obstetrics and gynecology residency, he moved to the United States in 1997 and completed another obstetrics and gynecology residency at the University at Buffalo in New York. He completed four fellowships in reproductive sciences, reproductive genetics, and reproductive endocrinology and infertility at West Virginia University, the University of Pittsburgh, the University of Toronto, and Wayne State University. He has published more than 150 peer-reviewed articles and reviews for several scientific journals. His honors include awards from the Society for Reproductive Endocrinology and Infertility, the Jerome J. Hoffman Award from the American Association of Gynecologic Laparoscopists, the Pacific Coast Reproductive Society (PCRS) Outstanding Paper Award, and the Society for Gynecologic Investigation President’s Presenter Award. Dr. Mitwally has been invited to speak at scientific meetings in the United States, Canada, the United Kingdom, China, Switzerland, India, and other countries. His expertise includes assisted reproduction (IVF), PCOS, pelvic pain and endometriosis, fertility preservation for patients with cancer, and reproductive genetics.

    Absolutely the best fertility doctor in the country. Believe me, my wife and I tried other clinics in Bexar County, where they treated you like a number and wanted your money. This is the only doctor who gives you his cell number and responds to texts. My wife and I tried for many years but could not conceive naturally. We were giving up when we met him, his wife, and the staff. He worked with us because we had spent all our money at other clinics. Do not believe the few negative reviews. He does recommend supplements for your wife, as other clinics do. They are not cheap, but they cost less than elsewhere. Yes, he currently works out of state and conducts virtual calls, but his assistant knows what she is doing. Their copayment arrangement and additional payments will help you reach delivery. We will always be grateful to Mitwally. My wife delivered our twins, a boy and a girl. Thank you, Mitwally.

    Dr. Mitwally and his clinic have everything I look for. They are personable, take time to learn about you and your fertility history, consider your views and feelings when deciding next steps, and care about their patients. You are not just a number here! You can text or call the doctor or clinic for quick help, and they can arrange a prompt appointment if you have concerns. Dr. Mitwally is very knowledgeable about medications and processes, and he and his staff operate very professionally. They also do not cost a fortune. I cannot speak about IUI or IVF, but from my appointments and what I have heard from patients with more treatment cycles, Dr. Mitwally avoids much of the overhead of large clinics and offices. You will not find a better doctor or clinic, and I would repeatedly recommend him to anyone seeking a fertility center!

    I am still trying to conceive, but I am very grateful to him and his team: professional, courteous, prompt, and thoughtful. I appreciate all the calls and messages back and forth. Infertility is a roller coaster, and their help has made the journey less frightening.

    Dr. Metwally and his team have been very good to my husband and me. We come from the Valley, so thank God they can see us on Saturdays. We appreciate that the doctor is only a text message or phone call away. We are still trying to become pregnant, but we trust God first and also trust Dr. Metwally. Thank you all.

    We love Dr. Mitwally and his team! We struggled with infertility for nearly ten years. We had been at another clinic, but by chance my husband walked into Dr. Mitwally’s office, and we finally welcomed our miracle baby. I still remember meeting the doctor for the first time. After hearing our story, he said, “God willing, you will have your own child.” Those words touched me deeply, and I am so grateful God brought him into our lives.     Mittwally Fertility Clinic Dr. Mittwally is an excellent doctor—knowledgeable, highly respected, and professional. I sincerely appreciate his expertise and attentive care. However, I was very dissatisfied with the clinic's front desk service. I called for nearly two weeks without receiving an adequate response, and the staff who answered were arrogant and unhelpful. This made communication extremely frustrating and stood in sharp contrast to the excellent medical care provided by the doctor.

    I would strongly warn anyone considering this clinic to think very carefully before proceeding. This place lacks basic monitoring, availability, and accountability. Fertility treatment requires close follow-up and responsiveness, yet this clinic is only accessible on very limited days, making proper tracking extremely difficult and unsafe. I was given multiple injections without clear explanations or individualized adjustments, despite repeatedly raising concerns. When I later experienced alarming symptoms, there was no meaningful follow-up, guidance, or support. Communication felt transactional — quick only when scheduling paid telemedicine, and nonexistent when real concerns were raised. Billing practices were equally troubling. I paid out of pocket for telemedicine, while a much higher charge was submitted to my insurance for the same service. Multiple attempts to get clarification were ignored. This experience caused unnecessary physical distress, emotional harm, and financial stress at a very vulnerable time. Patients seeking fertility care deserve careful monitoring, transparency, and compassion. Based on my experience, this clinic failed on all three. Please be cautious and ask very detailed questions before trusting this place with your health.

    My experience at this clinic was terrible and affected me both physically and emotionally. Although I repeatedly raised concerns about injection timing and monitoring, the clinic still administered multiple ovarian stimulation medications and HCG. I felt that my physical responses and symptoms were not taken seriously, and when I tried to voice my concerns, there was no appropriate follow-up or adjustment. The clinic's limited hours made matters worse. It is open only on certain days, making effective monitoring impossible, and contacting the doctor directly is extremely difficult. Communication seemed possible only after scheduling and paying for a telemedicine appointment. After this experience, I was left to manage severe symptoms and immense stress alone, without any guidance or support from the clinic. There were also serious billing issues. I paid out of pocket for telemedicine, yet the clinic submitted a higher charge to my insurance for the same service. Despite repeated attempts to ask about it, I received no response. I was emotionally vulnerable at the time, and the lack of care, follow-up, and transparency made matters even worse. I strongly advise other patients to ask detailed questions and ensure close monitoring before starting treatment.
