News | Study Links the Mediterranean Diet to Male Reproductive Health


A systematic review published in Frontiers in Nutrition suggests that the Mediterranean diet may improve male reproductive health, particularly semen quality. Conducted by researchers in Spain, the study examined its effects on semen quality in men of reproductive age.

Background

With about 15% of the global population—roughly 70 million couples of reproductive age—facing infertility, fertility research has become a major field. World Health Organization surveys indicate that male factors contribute to about half of infertility cases in couples, challenging the traditional view that infertility is solely a female issue. Major causes of male infertility include impaired spermatogenesis, idiopathic factors, endocrine disorders, and reduced sperm motility.


The Mediterranean diet is widely recognized for potential health benefits. Its anti-inflammatory and antioxidant properties may support male reproductive health, particularly semen quality. Evidence suggests that following this diet may reduce chronic disease risk and enhance male fertility by improving metabolic factors.


Methods

Researchers searched PubMed, the Cochrane Library, Scopus, and Web of Science. They screened open-access English- and Spanish-language articles published from 2012 to 2022 and included 10 eligible studies with 2,032 participants from several countries, mainly Spain.


Study quality was assessed with the Cochrane risk-of-bias tool, and evidence was graded using GRADE. The team focused on relationships among nutritional status, dietary patterns, and semen quality.


Results

Six of the 10 studies found a positive association between the Mediterranean diet and semen measures such as sperm concentration, motility, and total count. Three found significantly better semen quality among men who followed the diet closely. Two studies found no significant association.


Although the review found evidence of a positive association, the small samples, observational designs, and limited representativeness mean that further research is needed to confirm how broadly the findings apply.


Conclusion

Healthy eating patterns, especially the Mediterranean diet, may improve semen quality in men of reproductive age. Rich in polyunsaturated fatty acids, antioxidants, and anti-inflammatory compounds, the diet may protect sperm by reducing oxidative stress. The finding highlights the value of dietary guidance for couples planning pregnancy or using assisted reproductive technology. Further research is needed to clarify the relationship and inform strategies to improve fertility and health outcomes.
