Drs. Schnorr, Cook, McLaughlin, Riestenberg, and Peterson are board-certified in reproductive endocrinology and obstetrics and gynecology. Together, they have more than 70 years of clinical experience at leading U.S. centers. They have performed more than 35,000 ovulation induction and intrauterine insemination cycles and 15,000 IVF and donor egg cycles, with pregnancy rates among the highest in the country. Drs.     I struggled with infertility for a long time. After being referred to Coastal for a hysterosalpingogram (HSG) to check for blocked fallopian tubes, I felt at ease as soon as I met Dr. Riestenberg. I knew I had stage IV endometriosis, so we decided to retrieve eggs first, then perform excision surgery (including opening the blocked tubes), followed by a frozen embryo transfer (FET). Thankfully, our first attempt succeeded, and we now feel incredibly fortunate as we await our miracle baby! Dr. Riestenberg made me feel relaxed and never judged or dismissed. She patiently listened to my questions and concerns throughout the process. The nurses were also wonderful, and I am truly grateful to them! My only caution is that even if your fertility insurance provides “100% coverage,” many items may not be covered. I sometimes received unexpected bills, so I recommend reviewing your plan and cost estimate beforehand.

    I am writing this review to formally document my experience at Coastal Fertility Specialists (Nexton), specifically the denial of treatment that I believe lacked medical justification and conflicted with current evidence-based reproductive medicine. My expectations for care are informed by NaProTechnology, a science-based, physician-directed approach to fertility treatment that emphasizes identifying and treating underlying causes through detailed cycle monitoring, hormonal assessment, and targeted bioidentical hormone support. Although there are no locally certified NaPro physicians, its medical standards have been peer reviewed and are widely recognized. From my first visit, I encountered resistance to individualized evaluation. The clinic refused to review my laboratory results from the previous five years, when I was neither postpartum nor breastfeeding, even though this is standard practice in reproductive care for establishing baseline hormone function and diagnosing luteal phase defects. My concerns about painful periods since adolescence were dismissed without further investigation. My current physician, who supports restorative reproductive principles, has arranged an evaluation for endometriosis. Endometriosis is known to affect progesterone production, implantation, and fertility outcomes. Coastal Fertility refused to pursue or consider this diagnostic path. The clinic tested my serum iron level but did not test ferritin. Ferritin is the appropriate measure of iron stores and has significant clinical relevance to fertility, implantation, and menstrual health. Ferritin deficiency can occur even when iron levels appear normal. My subclinical polycystic ovary syndrome and subclinical hypothyroidism did not receive a comprehensive endocrine evaluation, and I was told the cause of my progesterone deficiency could not be determined. This conflicts with restorative reproductive medicine, which emphasizes identifying and treating luteal phase dysfunction rather than dismissing it as unexplained. I was also denied ongoing progesterone monitoring, despite progesterone being essential for implantation and early pregnancy support. While using 600 mg of vaginal progesterone, my Peak+3/4 level was only 7.9 ng/mL, well below the generally accepted minimum threshold associated with successful implantation and early pregnancy support, approximately 10 ng/mL. This strongly suggested an absorption issue, but it was not addressed. The physician strongly urged me to stop breastfeeding to continue treatment. I was later told that clinic policy supposedly denies fertility treatment to patients who are breastfeeding and less than one year postpartum. This policy was not disclosed at my initial visit and is inconsistent with current evidence. Fertility can return within months after delivery. In my case, my menstrual cycles typically return at 6 weeks postpartum, indicating early recovery of endocrine function. Restorative fertility approaches, including NaProTechnology, support evaluation and treatment once regular menstrual cycles resume, including for breastfeeding women. The physician also questioned the use of bioidentical progesterone during breastfeeding, even at therapeutic doses. However, evidence shows that very little bioidentical progesterone enters breast milk and that no harm to infants or milk production has been identified. It is routinely used in postpartum restorative fertility treatment. This review is intended to show that patients with hormone deficiencies, painful periods, recurrent miscarriage, or postpartum fertility concerns may encounter rigid protocols instead of individualized, evidence-based care. Patients deserve comprehensive evaluation, appropriate laboratory testing, and treatment that reflects current reproductive science, not blanket policies or selective application of research. I hope this feedback encourages greater transparency and a more comprehensive approach to care in the future.

    I do not want other patients to be misled. A clinic having credentials does not mean it is always right—advocate for yourself. Coastal Fertility's claim that delaying conception or stopping treatment was ‘to avoid harming me’ is not supported by current reproductive endocrinology. Once ovulatory cycles resume, fertility evaluation and treatment can safely begin, even during breastfeeding. - McNeilly, Human Reproduction Update, 2001 - American Society for Reproductive Medicine (ASRM) Practice Committee - American College of Obstetricians and Gynecologists (ACOG) Committee Opinion Bioidentical progesterone supplementation is safe during breastfeeding, transfers into breast milk only minimally, and has no known harm to the infant or lactation. - Di Renzo et al., American Journal of Obstetrics and Gynecology, 2005 - Wren et al., Fertility and Sterility, 2012 - LactMed database; Hale & Rowe, Medications and Mothers' Milk NaProTechnology and restorative reproductive medicine emphasize cycle-based hormone monitoring, early post-ovulation progesterone supplementation, and individualized dosing for luteal phase defects and recurrent miscarriage. —Hilgers, NaProTechnology: A Clinician's Guide, 2015 Making patients wait several months or even a full year postpartum, or denying treatment based on policy rather than physiology, exposes them to unnecessary risk rather than ‘protecting’ them. More facts, fewer ‘feelings’ Timing of policy disclosure: If a clinic requires patients to wait until one year postpartum or until they stop breastfeeding before receiving progesterone support, this should be disclosed at the first visit. Be sure to bring a support person to your initial consultation so there can be no later dispute about what was or was not disclosed. Record the visit if possible. South Carolina is a one-party consent state. Waiting until one or two treatment cycles later to tell a patient is a serious omission. It forces patients into limbo for months, delays care, and exposes them to unnecessary risk. This delay is not trivial; it has real clinical consequences. In short, this is withholding or omitting material information, which is ethically problematic because it affects a patient's ability to make timely, informed treatment decisions. Inconsistent enforcement: A friend of mine received progesterone support at the same clinic six years ago, although at a different location, raising the question of whether this is truly clinic policy or an individual physician's preference. If I had to guess, it is the latter. Inconsistent enforcement suggests arbitrariness, undermines trust, and prevents patients from making informed decisions.

    My husband and I are expecting our first baby, and this has been an amazing experience with so many wonderful people. Dr. Reistenberg was a true blessing to us. She patiently guided us through every step, even when I was nervous. She was warm and approachable and answered all our questions, which I greatly appreciated.     After struggling with secondary infertility, I decided to contact Coastal. Dr. Reistenberg and our nurse Lauren were wonderful. Our first IVF cycle produced no viable embryos, so Dr. R changed my protocol, and the second cycle was a complete turnaround. We ultimately obtained 5 blastocysts, compared with 0 the first time. Dr. Reistenberg is very compassionate and advises you as she would a friend or sister. Lauren patiently answered every question, listened carefully, and always responded promptly to emails. I had two egg retrievals at the Mt. Pleasant clinic, and the staff seemed somewhat mechanical both times, but I work in surgery and understand. It was not a major issue. I am now 14 weeks pregnant with our rainbow baby. I am incredibly grateful to Coastal Fertility Specialists for helping complete our little family. I will always be grateful from the bottom of my heart. You are the best!

    Coastal Fertility gave me two wonderful sons, Shelton (3) and Theodore (1), and the baby girl I am now carrying, due in November. Dr. McLaughlin is amazing. She is very gentle and fully present during appointments and conversations. Alex always kept me updated. Everyone throughout the clinic, including ultrasound and laboratory staff, was always smiling and very friendly. I cannot remember her name, but one staff member performed my ultrasound and laboratory work at the Nexton location almost every time. She was always gentle and soft-spoken and made me feel valued.     I love this facility and its staff! My husband and I tried to conceive without success and decided to see a fertility specialist to determine what was happening. Dr. Peterson and Nurse Alex are truly a dream team. They eased all my anxiety while providing excellent, attentive care. Alex promptly answered my countless emails, and Dr. Peterson never made me feel afraid or anxious. I am officially four months pregnant with my baby boy, and I cannot thank them enough!     Dr. Risterberg and Nurse Alex were the best team I could have asked for to help bring our second angel into the world. The care and compassion at the Nexton office are exceptional! My first and second IVF cycles at the Mount Pleasant office felt very robotic, almost like an assembly line. My previous doctor did not seem to notice or care that we had already experienced a failed transfer when giving exactly the same speech before our second transfer. I felt extremely discouraged and like just a number. Dr. R, Alex, and the entire Nexton front-desk team restored my confidence in the practice and made me feel more welcome and special during my third transfer. I recommend them to everyone I know who is beginning IVF!

    A clean office and transparent pricing for outside monitoring services. However, I spent six months trying to obtain a refund for an overpayment. It was only $25, but they stopped responding. If you are a customer, I recommend avoiding advance payment whenever possible because any refund owed to you may not be returned. I am simply glad they did not owe me more. I will update my rating if they respond to my repeated follow-ups or I receive the check. Edit: Although the employee with whom I exchanged emails about this check never responded again, I called the number in the reply to this review. I do not know what happened or why the original attempt failed, but the employee who answered reissued the check. I have updated the rating to five stars despite the refund issue because they provide outside monitoring. Thank you.
