Guide | Back Labor: A Hidden Challenge During Childbirth and Ways to Cope

Many women experience severe lower-back pain during childbirth, known as back labor. Although it does not directly endanger the mother or baby, it can make labor more uncomfortable. Understanding its causes and ways to prevent or relieve it can help expectant mothers prepare.

What is back labor?

Back labor is intense lower-back pain during labor. It may persist between contractions and worsen during them, sometimes with continuous cramping. Studies indicate that about 15%-32% of women experience lower-back pain before labor, and 5%-8% experience back labor throughout childbirth.

What causes back labor?

The main cause is the fetus's position in the uterus.

Occiput posterior position: If the fetus faces the mother's abdomen, its head may press directly on her tailbone and cause back labor.

Pelvic shape: The shape and size of the mother's pelvis may restrict fetal movement and increase the risk of back pain.

Muscles and ligaments: Tight or weak pelvic muscles or ligaments may prevent the fetus from moving into the optimal position for birth.

Posture: Poor posture can tilt the pelvis forward or backward and increase the likelihood of back labor.

Research also suggests that women who regularly have back pain during menstruation are more likely to experience back labor regardless of fetal position.

Does back labor affect fetal health?

Back labor itself does not directly harm the fetus, but a persistently unfavorable fetal position may cause:

Longer labor and maternal fatigue;

A need for forceps or vacuum-assisted delivery;

A higher likelihood of episiotomy or cesarean delivery.

Most fetuses naturally rotate into a better position during labor or can be assisted by the medical team.

Ways to help prevent back labor

It cannot always be avoided, but these measures may lower the likelihood:

Activity during pregnancy: Use an exercise ball daily or do moderate exercise to encourage an optimal fetal position.

Proper sitting posture: Avoid deep sofas and recliners, and sit with the knees lower than the hips.

Prenatal care: Chiropractic care or prenatal massage may improve pelvic alignment.

Position changes during labor: Avoid lying flat on the back; try side-lying or tilted positions.

Standing and movement: Walk, squat, or lunge during labor when possible, using gravity to help the fetus rotate.

Ways to relieve back labor

The following may help during labor:

Hot or cold packs on the lower back;

Massage or counterpressure;

A warm bath or shower;

Pressure with a tennis ball or roller;

A heat pack, such as a sock filled with warm rice;

Pain medication, including an epidural.

Expert advice

Expectant mothers should monitor fetal position during labor and discuss concerns with the medical team. Appropriate care and labor strategies can help most women deliver safely.
