A study found that only about 4% of men who apply to become sperm donors in the United States and Denmark complete all screening and reach the sperm-freezing stage for use in medically assisted reproduction. The study provides the first systematic public account of screening standards and processes within the sperm-bank industry and highlights the growing shortage of donors.
The study was led by Dr. Allan Pacey, professor of andrology at the University of Sheffield in the United Kingdom, and published this month in Human Reproduction.
Professor Pacey said that although the 4% completion rate may appear low, it did not surprise him. He previously operated a small sperm bank in the United Kingdom, where fewer than 4 in every 100 applicants passed all screening. He emphasized, however, that the low acceptance rate should not discourage men who are interested in donating: "Sperm banks always need a steady supply of new applicants."
The study analyzed all men who applied to the internationally known sperm bank Cryos International in the United States and Denmark between 2018 and 2019, a total sample of 11,702. Although most completed online registration or an initial semen test, most did not continue through later stages. Reasons included poor sperm quality after freezing, self-reported health problems, unsuccessful infectious-disease screening, or not meeting genetic screening criteria.
Dr. Michael Thomas, president of the American Society for Reproductive Medicine (ASRM) and chair of the Department of Obstetrics and Gynecology at the University of Cincinnati Medical Center, was surprised by the figure. "4% is extremely low, far below the 20%-30% acceptance rate I previously told potential donors," he said. This suggests that leading sperm banks now apply increasingly strict criteria and that donation is no longer as simple as a college student walking in to earn fifty dollars for beer.
The study also found that identity-release donors were more likely to complete the process: 4.7% did so, compared with 3.2% of anonymous donors. Pacey said it was encouraging that some men who initially chose anonymity agreed during the process to become identifiable. This is particularly important for patients in the United Kingdom, where the law gives donor-conceived children the right to learn the donor's identity in the future.
Completion rates also differed substantially by country: 6% of Danish men were ultimately accepted, compared with only 1% of U.S. men. Pacey hopes to investigate the cultural and procedural reasons for these differences. "Adapting the donation process to different countries may encourage more people to participate."
The study also highlighted another trend: the number of sperm donors is declining worldwide. This creates challenges for people who rely on donor sperm to build a family, including same-sex couples, single women, and heterosexual couples unable to conceive without assistance because of male infertility or genetic concerns.

Dr. Thomas said, "More donor-conceived people are using genetic testing to find relatives and ask questions about their genetic background." Many anonymous donors who once believed they would never be identified are now realizing that their identities may eventually be discovered.
Pacey concluded that public understanding of sperm donation needs to improve and that misconceptions and stigma should be reduced. The team's next step is to analyze why so many men withdraw after applying and to improve the process to encourage potential donors to continue.
