As more families achieve parenthood with the help of infertility treatment, the relationship between fertility challenges, infertility treatment, and long-term child development is drawing increasing attention. Recently, a new study funded by the National Institutes of Health's Environmental Influences on Child Health Outcomes (ECHO) program showed that parental difficulty conceiving may be associated with small differences in child behavioral and developmental outcomes; in contrast, the study found no clear association between in vitro fertilization itself and child neurodevelopmental outcomes.

The study was published in JAMA Network Open, titled "Subfecundity, Infertility Treatment, and Child Neurodevelopment." Based on ECHO cohort data, the research team included 15,382 mother-child pairs from 44 study sites across the United States and Puerto Rico, analyzing the relationships between parental fertility history, infertility treatment methods, and child neurodevelopmental outcomes. Child developmental assessments covered ages 2 to 10 years, based primarily on parent-completed questionnaires and clinician diagnoses of autism spectrum disorder and attention-deficit/hyperactivity disorder.


"Difficulty conceiving" in the study was not limited to those who had already received infertility treatment. The researchers defined it as a history of infertility consultation, diagnosis, or treatment, experiencing at least two prior miscarriages, or having unprotected heterosexual intercourse for 12 months without achieving pregnancy. The study also divided infertility treatment into in vitro fertilization and non-in vitro fertilization treatment, in order to distinguish the potential effects of treatment methods from those of the underlying fertility problems themselves.


The study results showed that, compared with families without a history of difficulty conceiving, children of parents with a history of difficulty conceiving had slightly higher behavioral problem scores, slightly more autism-like traits, and a higher likelihood of being diagnosed with autism spectrum disorder. Notably, similar associations were observed even when these children were conceived naturally. This result suggests that child developmental differences may not primarily arise from infertility treatment itself, but may be more likely related to underlying health factors, genetic factors, environmental factors, or other mechanisms not yet fully understood that contribute to difficulty conceiving.


Regarding treatment methods, the study found that children conceived through non-in vitro fertilization infertility treatment had a higher likelihood of being diagnosed with attention-deficit/hyperactivity disorder compared with naturally conceived children. However, the researchers also noted that this finding requires further investigation to explain, particularly to clarify whether specific treatment indications, underlying diseases, and related health conditions among those receiving non-in vitro fertilization treatment contribute to this association.


In contrast, the study found no association between in vitro fertilization itself and child neurodevelopmental outcomes supported by clear evidence. The researchers believe this result is consistent with growing evidence: infertility treatment itself may not necessarily be an independent cause of child neurodevelopmental differences, and what truly warrants attention may be the pre-existing fertility difficulties and their underlying biological and environmental factors.


Study author Dr. Linda Kahn, an ECHO researcher at NYU Langone Health, stated that this study further supports an emerging consensus that infertility treatment itself does not appear to independently cause child neurodevelopmental problems. Rather, parents' pre-existing fertility problems, whether their origins are genetic, environmental, or other contributing factors, may be the primary drivers of the observed associations.


Child neurodevelopmental differences, including behavioral problems, autism spectrum disorder, and attention-deficit/hyperactivity disorder, may affect children's long-term health, learning ability, and social functioning. Therefore, clarifying the respective roles that infertility treatment and underlying fertility problems may play is of significant importance for clinical counseling and family decision-making. Past related studies often had small sample sizes or found it difficult to separate the effects of infertility itself from those of treatment; the large-sample, multicenter data from the ECHO cohort provides a stronger analytical foundation for this question.


However, the researchers also emphasized that the results should be interpreted with caution. First, the behavioral and developmental differences observed in the study were generally small and may not necessarily have clear clinical significance. Second, this is an observational cohort study that can suggest associations but cannot prove causation. The factors affecting child neurodevelopment are highly complex and may involve parental genetic background, prenatal environment, preterm birth, birth weight, family environment, socioeconomic factors, and multiple exposures during child growth.


The study also showed that the associations between difficulty conceiving and some behavioral scores and autism-like traits may be influenced by factors such as gestational age at birth. In other words, some pregnancy and birth outcomes that are more common among those with difficulty conceiving may play a mediating role in children's subsequent developmental performance. However, for autism spectrum disorder diagnoses, the study did not find that gestational age at birth could fully explain the observed associations, suggesting that the underlying mechanisms still require deeper exploration.


For families considering or already receiving infertility treatment, this study does not mean that infertility treatment will directly cause child developmental problems. Rather, it reminds the clinical community that, during fertility counseling, greater attention should be paid to parents' baseline health status, causes of fertility difficulties, pregnancy management, and early childhood developmental screening. For families with a history of difficulty conceiving, strengthening preconception assessment, pregnancy management, and early childhood behavioral and developmental monitoring may help identify and intervene in potential problems earlier.


The research team stated that future studies need to further distinguish between different causes of difficulty conceiving and different treatment indications, clarify which underlying factors may be associated with child neurodevelopmental differences, and explore the relevant biological mechanisms. As the utilization rate of assisted reproduction and infertility treatment rises, establishing more refined and longer-term follow-up evidence will help provide families with more accurate, balanced, and individualized medical advice.
